Modern Age will offer a vertically integrated telemedicine and treatment platform that provides an array of clinical and non-clinical services designed to help patients lower their subjective age.

Full & Part-time availability.

Primary Responsibilities:

• Maintain high clinical standard of service through adherence to all clinical protocols
• Administer blood draws and prepare labs
• Administer ultrasound bone density scans
• Develop appropriate treatment plans based on customer consultations, lab results and scan results
• Assist with scheduling of patients' consultations and treatments
• Ensure customer safety when performing treatments
• Maintain charting/clinical documentation for each customer
• Prescribe appropriate pharmacologic and non-pharmacologic treatments according to approved formulary
• Complete consultations (in-person and via video/chat) and respond to patient messages within identified timeframe guidelines

Required Qualifications

• 3+ years experience working in an outpatient, Functional Medicine setting
• Experience with EMR software required
• Must be a currently licensed Nurse Practitioner in the state of New York; other states a plus
• Must maintain yearly continued education and stay current on product and service lines
• Experience working in a multi-specialty environment a plus
