Our client partners with physician groups and home care agencies to help implement and deliver these services; providing administrative support, and helping to find committed and capable staff for the physician group.

What you will do :

• Receive clinical calls and triage
• Utilize telehealth system and perform virtual visits 
• Coordinate care appropriately and timely with members of the care team both internal and external
• Have the ability to educate members, family or other caregivers on chronic conditions, diet changes, and pieces of their care plan  
• Have confidence in the ability to triage appropriately and escalate to appropriate resources
• Utilize technology for documentation

Minimum Qualifications :

• Fluency in English and Spanish in writing, reading, and speaking (required)
• Graduated from an accredited nursing program (required)
• Current RN License in good standing in the state of NY (required)
• At least 2 years of nursing experience providing care to adult and geriatric patient populations (required)
• Confidence with clinical skills in the performance of telephonic triage/assessment (required)
• The ability to work remotely and has a private area in their home/workspace (required)
• A genuine, compassionate desire to serve others and help those in need
• High-speed home WiFi/data connection to support company-provided IT equipment
