Position Details:

• Seeking a Case Manager RN for a traveler-friendly facility in Glens Falls, NY.
• While Basic Life Support (BLS) certification is preferred, other qualifications are also considered.
• The requirement includes a recent 3-year experience in Case Management.
• The role is focused on Registered Nurse Case Management within medical/surgical and/or telemetry units.
• Responsibilities encompass active participation in Multidisciplinary Rounds, harmonious collaboration with Hospitalist and Nursing Teams, and handling a caseload of 20-23 RN CM to meet organizational objectives.
• Preference for New York State (NYS) PRI certification.
• Additional experience in Utilization Management (UM) is beneficial.

Unit Details:

• Documentation is managed using the Cerner charting system.
• A Registered Nurse Case Management role, primarily for medical/surgical and/or telemetry units.
• The focus of this position is to oversee exit portal throughput.
• Responsibilities involve expediting discharges from the facility once patients are clinically stabilized for a lower level of care.
• Active participation in Multidisciplinary Rounds is expected.
• Collaboration with Hospitalist and Nursing Teams is integral to this role.
• Managing a caseload of 20-23 RN CM cases is necessary to achieve organizational goals.
