The registered nurse will provide primary and urgent care services to adults, according to certification at our homeless medical respite program.
Responsibilities:
- Supports, leads, and directs clinical/operational processes in the Medical Respite
- Supports medical facilities, Admissions Manager and other referral sources in assessing and accepting referrals into the respite
- Provides education and assessment to patients looking to access MAT programs
- Provide medication management using specified guidelines outlined by the medical respite standards
- Observes ambulatory care guidelines for infection control (handwashing, care of environment, use of safety syringes and sharps containers, use of personal protective equipment)
- Evaluates and treats patients in accordance with clinical best practices for post-acute care in a medical respite care setting
- In collaboration with the case manager, connects clients to primary care, behavioral health, family violence or other social services as needed
- Provides client-centered health services that incorporate self-management plans
- Provide ongoing management, education, and support of patients in all phases of treatment including scheduled nursing visits medication teaching, monitoring, pill counts, observed dosing, and medication refills
- Exhibit non-judgmental, empathetic and supportive approach when communicating with patients/families and staff
- Incorporates evidence-based practices as appropriate for the individual patient
- Works within all state and federal laws and regulations and the agreement with the collaborative physician
- Administers medications and manages acute and chronic health problems using evidence-based protocols
- Notifies immediate supervisor of significant problems
- Provides patient/education, health counseling, and health advocacy services to patients and significant others. Be willing to work evenings and weekends, as deemed necessary
- Provide on-call support
- Maintains positive relationships with funding sources and community organizations
- Completes appropriate documentation with record keeping
- Patient contact: Approximately 70% of the time
- Consistently use clear, concise, and effective written and oral communication; and comply with departmental policies when sharing/documenting relevant patient care data
- Perform other related tasks and duties, as directed and assigned
- Act in the capacity of a mandated reporter to both the State of Pennsylvania and the Joint Commission
- Perform additional duties as outlined by Supervisor
Requirements:
- Certified as a Registered Nurse in the State of Pennsylvania
- Experience in primary and/or care, community health preferred
- At least three (3) years of nursing experience
- Keeps abreast of all pertinent federal, state regulations, laws, and policies as they presently exist and as they change or are modified
- Comply with HIPAA and Confidentiality Policies and Procedures as they apply to the job
- Comply with Department of Public Health (DPH), The Joint Commission and other accreditation and regulatory agencies standards
- Knowledge and adherence to Infection Control and Environment of Care Guidelines and Procedures as described in the annual education module
