Job Summary: We are seeking a dedicated and compassionate Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will be responsible for conducting thorough risk assessments in the homes of patients, ensuring their safety and well-being. Your expertise in healthcare, clinical assessment, and communication skills will be critical in identifying potential risks and developing personalized care plans to mitigate them
Responsibilities:
- Conduct comprehensive in-home assessments to identify potential health and safety risks for patients
- Evaluate the patient's medical history, current health status, and home environment to assess risk factors
- Identify potential hazards such as fall risks, medication management issues, and environmental safety concerns
- Collaborate with patients, their families, and healthcare providers to develop individualized care plans to reduce identified risks
- Provide education and guidance to patients and their families on safety measures and strategies to minimize risks
- Coordinate with healthcare professionals, social workers, and community resources to support the implementation of care plans
- Document assessment findings, care plans, and progress notes accurately and in a timely manner
- Stay current on best practices in home risk assessment and healthcare regulations
- Maintain patient confidentiality and adhere to ethical and professional standards
Qualifications:
- Current Nurse Practitioner (NP) license and certification
- Master's or Doctoral degree in Nursing
- Proven experience in healthcare, with preference given to those with previous home care or geriatric experience
- Excellent clinical assessment skills
- Strong communication and interpersonal abilities
- Empathy and compassion in dealing with patients and their families
- Ability to work independently and as part of a healthcare team
- Knowledge of healthcare regulations and safety guidelines
- Proficiency in electronic health record (EHR) systems
Working Conditions:
- This position involves travel to patients' homes within a designated service area
- May require working evenings or weekends based on patient needs
