Job Summary: We are seeking a dedicated and compassionate Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will be responsible for conducting thorough risk assessments in the homes of patients, ensuring their safety and well-being. Your expertise in healthcare, clinical assessment, and communication skills will be critical in identifying potential risks and developing personalized care plans to mitigate them

Responsibilities:

  • Conduct comprehensive in-home assessments to identify potential health and safety risks for patients
  • Evaluate the patient's medical history, current health status, and home environment to assess risk factors
  • Identify potential hazards such as fall risks, medication management issues, and environmental safety concerns
  • Collaborate with patients, their families, and healthcare providers to develop individualized care plans to reduce identified risks
  • Provide education and guidance to patients and their families on safety measures and strategies to minimize risks
  • Coordinate with healthcare professionals, social workers, and community resources to support the implementation of care plans
  • Document assessment findings, care plans, and progress notes accurately and in a timely manner
  • Stay current on best practices in home risk assessment and healthcare regulations
  • Maintain patient confidentiality and adhere to ethical and professional standards

Qualifications:

  • Current Nurse Practitioner (NP) license and certification
  • Master's or Doctoral degree in Nursing
  • Proven experience in healthcare, with preference given to those with previous home care or geriatric experience
  • Excellent clinical assessment skills
  • Strong communication and interpersonal abilities
  • Empathy and compassion in dealing with patients and their families
  • Ability to work independently and as part of a healthcare team
  • Knowledge of healthcare regulations and safety guidelines
  • Proficiency in electronic health record (EHR) systems

Working Conditions:
