Responsibilities:

• Assessment: You will conduct thorough assessments of patients' physical, social, behavioral, and mental status through interviews, direct observation, and medical record review.
• Planning: Based on your assessment findings and the patient's medical/surgical diagnosis, you will develop a personalized care plan that addresses their physical and psychological needs, as well as their educational level. You will also identify the need for referrals to other healthcare professionals.

Requirements & Qualifications:

• Education: Bachelor's degree in Nursing required.
• Experience: Minimum three (3) years of medical-surgical nursing experience preferred.
• Knowledge and Skills: Effective communication in English, verbally and in writing.

Licenses and Certifications:

• Current NY State RN License.
• Basic Cardiac Life Support (BCLS) certification required.
