Responsibilities:
- Monitor patient vitals and health indicators remotely, using state-of-the-art software and tools.
- Manage patients with chronic illnesses by providing telephonic coaching, guidance, and health education.
- Develop personalized care plans based on individual patient needs.
- Communicate regularly with patients, care teams, and physicians to ensure optimal patient outcomes.
- Document and maintain accurate, comprehensive patient records.
- Enroll patients in virtual care services.
Requirements & Qualifications:
- License: Licensed Practical Nurse (LPN) or Registered Nurse (RN) License.
- Experience: Minimum of 2 years of nursing experience, with a focus on chronic disease management.
- Technical Skills: Proficiency in using medical software and digital health tools.
- Communication Skills: Excellent communication, interpersonal, and patient management skills. Bilingual in Spanish
- Independence: Ability to work independently and handle multiple tasks simultaneously in a remote environment.
- Commitment: Strong commitment to patient care and privacy protocols.
