Overview
Under direct supervision of a Registered Nurse or physician, the LPN performs nursing duties in the care and treatment of patients on an assigned unit. The duties require the use of tact and judgment in the care and treatment of patients. May assist in the supervision of non-professional subordinates. Is cognizant of philosophy, standards, objectives and policies of the Nursing Department. Performs related duties as required.
Responsibilities
- Demonstrates proficient knowledge in the treatment of patients requiring nursing care. Uses available diagnostic results and other information to make an accurate evaluation.
- Applies knowledge of illnesses, injuries, and diseases and uses available resources in performing the nursing process.
- Performs timely, comprehensive, and accurate reevaluations and documentation at regular intervals as defined by the department, when a significant change occurs in the resident's condition, and when otherwise warranted.
- Is considerate of residents' privacy during the evaluation process. Contributes to an individualized and appropriate plan of care delineating measurable patient care goals
- Contributes to the plan of care making necessary changes as the condition of the patient indicates. Bases care decisions on resident needs and care priorities. Monitors patient outcomes.
- Monitor continued care needs for discharge and coordinates resources to meet post-institutional requirement.
- Collaborate, as appropriate, with Care Manager, physician, and other clinical disciplines in planning interventions to meet resident and family needs in a timely manner.
Qualifications
Education:
- Graduation from an accredited school of practical nursing or completion of equivalent study in an approved program in professional nursing.
Experience:
- Minimum one (1) year clinical experience preferred.
Knowledge and Skills:
- Ability to work in surroundings with degree of dust, noise, etc. without undue discomfort
- Ability to hear, talk, walk, stand, bend and stretch
- Ability to read, write and interpret documents
- Ability to sit for long periods of time at desk to perform job functions
- Ability to stand for long periods of time at required patient bedside and/or procedural areas
- Ability to communicate effectively with patients and/or co-workers
- May be exposed to mechanical/electrical/radiant/explosive/burn and chemical hazards
- May be required to handle, dispose of and/or identify hazardous waste and hazardous materials
- May be required to operate and clean equipment
- May be required to handle/ assist in lifting patients onto/out of exam beds/procedure stations
- Must be trained in and knowledgeable of all Kingsbrook/Rutland policies and procedures pertaining to hazardous waste and hazardous materials and maintain qualifications applicable to assigned work
- Other: Knowledge of all codes and demonstrate understanding
Licenses, Certifications:
- Current NYS Licensure or the eligibility to be licensed to practice as a practical nurse.
Physical Requirements:
- Duties performed routinely require exposure to blood, body fluid and tissue.
- Normal routine involves no exposure to blood, body fluid or tissue, but potential for exposure.
