Key Responsibilities:

  • Conduct comprehensive psychosocial assessments to identify the needs, strengths, and challenges of patients and families.
  • Develop and implement treatment plans in collaboration with medical teams to address psychosocial, emotional, and financial needs.
  • Provide counseling and therapeutic interventions to patients and families coping with acute, chronic, or terminal illnesses.
  • Facilitate discharge planning by coordinating resources, services, and referrals to community agencies and support networks.
  • Advocate for patients' rights and ensure their access to necessary healthcare services.
  • Maintain accurate, confidential documentation of patient interactions and treatment plans.
  • Participate in multidisciplinary team meetings and contribute to patient care plans.
  • Stay up to date with social work best practices, legal requirements, and hospital policies.

Qualifications:

  • MUST have hospital discharge planning experience in a hospital setting
  • Master's Degree in Social Work (MSW) from an accredited program.
  • New York State Licensed Master Social Worker (LMSW) required.
  • Previous experience in a hospital, healthcare, or clinical setting preferred.
  • Strong knowledge of medical terminology, healthcare systems, and community resources.
  • Excellent interpersonal, communication, and organizational skills.
  • Ability to work effectively within a multidisciplinary team.
  • Ability to handle sensitive situations with empathy, professionalism, and confidentiality.

Shifts: Full Time. 9am-5pm. 5 days/week.
