A rapidly growing nationwide healthcare organization is offering FULL TIME VIRTUAL WORK for nurses (LVN/LPNs). Nurses will be under physician general supervision.
The company - an early leader in telehealth - engages geriatric patients who face multiple chronic conditions and geographical barriers to care.
As a Command Center Nurse, you'll work virtually to assess and evaluate patient needs to achieve quality service. You'll have the opportunity to employ a variety of care management strategies and processes.
By offering telephone and video visits, you can assess and support patient health, offer education, identify resources, and help remove barriers to achieving health and independence.
Job Description:
*Evaluate user needs and requirements to achieve and/or maintain optimal wellness state using the digital health platform
*Assess Medicare, Medicaid, and Health plan members holistically over phone and video
*Guide users and their caregivers toward, and facilitate interaction with, resources appropriate for user wellbeing
*Work in collaboration with a multidisciplinary team to assess user physical, environmental and psycho-social health issues. Employ a variety of techniques to give timely intervention.
*Provide patient education, support, and interventions to patients who have home physiologic equipment and monitoring.
*Escalate timely clinical assessments to physicians in a clear, concise manner
*Participate as a multidisciplinary team member and provide direction to other Care Command Center team members to maintain and improve user health at home
Requirements:
*Bilingual proficiency - English and Spanish - strongly preferred
*Demonstrated clinical knowledge and expertise in remote patient monitoring, telehealth, chronic conditions and individualized care planning experience are a plus.
*Active Registered Nurse (RN) with no disciplinary action.
*Experience with motivational interviewing.
*Experience with MCG or CMS guidelines, assessment and documentation practice.
*Minimum of three years of clinical nursing experience.
*Must possess telephone, typing, and virtual communication prowess
*Able to work 40 hours per week between the hours of 7am - 5pm PT with occasional Saturdays.
